I, WILLIAM ELLIOT LAWSON JOHNSTON, a Director of THE APOLLINARIS COMPANY,

LIMITED, of No. 4, Stratford Place, Oxford Street, in the County of London,

in England, hereby DECLARE as follows, that is to say:-

1. That the 70 Cases of Apollinaris Water, of which proof of shipment

per steamer "Bayern" from Antwerp 15th. July 1914 has been duly

submitted to The Under Secretary of State on the 20th. September

1915, were sent by The Apollinaris Company, Limited, of London, to

Messrs. Carlowitz and Company of Tientsin in China, for sale by the

said Carlowitz and Company on account of the said Shippers, The

Apollinaris Company, Limited, whose goods they would have remained

until sold in China by the said Carlowitz and Company, this latter

Company's sole interest in the said goods consisting of a commission

of 7 allowed to them by the said The Apollinaris Company, Limited,

after such sale.

2. That I know the copy of invoice hereunto annexed, and marked "A",

to be a true and correct copy of the original invoice dated 17th.

July 1914 sent by the said The Apollinaris Company, Limited, London,

to the said Carlowitz and Company, Tientsin, for the abovementioned

70 Cases of Apollinaris Water.

3. That I further know the copy of particulars of the 1 Case of

Advertising Material hereunto annexed, and marked "B", to be a true

and correct copy of the original list of details dated 17th. July

1914 andsent by the said The Apollinaris Company, Limited, to the

said Carlowitz and Company, Tientsin.

4. That the contents of the Case of Advertising Material, referred to

in the preceding paragraph, were sent by the said The Apollinaris

Company, Limited, to the said Carlowitz and Company, for

distribution gratis by the latter Company on behalf of the said The Apollinaris Company, Limited.
